Important: Please ensure EACH attendee buys a ticket individually (ie do not complete any purchase which combines more than one person per order). This will ensure we have each attendees details.

Our first major Youth Rally for 2018 is set to be a big one. The service will commence at 10.30am and lunch will be provided as you are sent out in groups to impact our city that afternoon. No one will be forced to do anything but we ask that you open your heart and step away from your comfort zone to allow God’s Spirit to move in you. Cars can be left at the rally venue as we go out to serve, we will be meeting back at Lake Monger to celebrate and enjoy dinner together.

EVENT TERMS & CONDITIONS FOR UNDER 18 ATTENDEES - PARENTAL CONSENT REQUIRED:

I am aware in accepting these terms & conditions for my child’s participation in the Youth Rally that certain elements of the program could be physically and emotionally demanding. Furthermore, I understand that certain inherent risks and dangers may exist in the activities. I acknowledge that while the WA Youth Dept and its leaders will make every reasonable effort to minimise exposure to known risks, all hazards and dangers associated with these activities cannot be foreseen or may be beyond the control of the WA Youth Dept, its leaders and staff. In the event of an emergency, and if the WA Youth Dept are unable to contact me:

• I authorise the leaders to obtain medical advice and/or assistance which they deem necessary.

• I further authorise qualified practitioners to administer anaesthetic if required.

• I accept all operation, blood transfusion and/or anaesthetic risks involved in the event that such procedures are deemed necessary.

• I accept the responsibility for payment and agree to pay medical, transport and any other related expenses.

• I confirm that the information contained in this application is true and correct.

• I agree to inform the directors of any changes to these details.

• I understand that the leaders will take all responsible care of my child whilst on site and that the WA Youth Department or it’s representatives will not be liable in any injury or accident, or for damage or loss of property. I understand that in cases of unacceptable behaviour, attendees will be sent home.

• Transportation to and from the even are the responsibility of the parent and/or attendee. Upon exiting the event, the WA Youth Department or it’s representatives no longer have care of duty for any attendee.

Photographs and video will be taken. The WA Youth Dept. and WA Conference will use these appropriately in the form of further advertising, NewsWest and places on our web page and social media. Please contact the Youth Dept if you have any concerns or wish to discuss this further 

Tickets must be printed and presented upon arrival at the venue. 

The WA Youth Department reserves the right to decline applicants.

Protecting your privacy is important to us. The information we seek allows us to manage risk, provide reasonable care and administrate your involvement in our program. We are careful to keep your information confidential, and provide it only to those agents acting on behalf of the WA Youth Dept. in accordance with Youth Rally programs. We will not use your information for other purposes. You’re welcome to contact our office (08) 9398 7222 in relation to issues regarding your personal information. We only ask for information which is necessary for the purposes outlined in this statement. If not all information is provided your child may miss the opportunity to be involved in our program.
